1.0 Clinical CE Hour | Recorded / Asynchronous Documentation is one of the most important clinical and risk-management responsibilities in social work practice. Inadequate, vague, or incomplete documentation can increase vulnerability to audits, reimbursement denials, licensing board complaints, legal scrutiny, and challenges to clinical decision-making. This 1.0-hour clinical continuing education course provides practical, evidence-informed guidance on creating defensible clinical documentation that supports medical necessity, demonstrates clinical reasoning, and reflects skilled professional practice. Participants will learn how to identify common documentation errors, strengthen progress notes, document risk assessment and safety planning, distinguish progress notes from psychotherapy notes, and apply documentation practices that align with ethical, regulatory, and payer expectations. The course also reviews commonly used documentation models, including SOAP, DAP, BIRP, and GIRP, while addressing documentation considerations related to telehealth, healthcare settings, supervision, artificial intelligence, measurement-based care, and Louisiana social work practice standards. Designed for social workers practicing in healthcare, hospice, behavioral health, schools, private practice, and community settings, this course emphasizes practical strategies that improve documentation quality, strengthen professional credibility, and reduce documentation-related risk.

Learning Levels

  • Learning Level: Intermediate This course is designed for licensed social workers who possess foundational knowledge of clinical documentation and professional recordkeeping. Participants will build upon existing documentation skills by examining high-risk documentation errors, strengthening medical necessity language, improving risk documentation practices, and applying evidence-informed strategies for defensible clinical writing. The course emphasizes practical application, clinical judgment, ethical responsibilities, and regulatory considerations relevant to contemporary social work practice.

    Donna Wimberly is a Licensed Clinical Social Worker and Board Approved Clinical Supervisor with extensive experience in clinical practice, leadership, and social work management. As graduate of Louisiana State University School of Social Work, owner of her own private practice, and Manager of Social Work Services, Donna strives to enhance clinical outcomes and foster team development while dedicating her career to advancing mental health and social work practice. Donna is a member of the National Association of Social Workers and has served as member of the NASW-Louisiana Board, the Regional Chair for the Lafayette Chapter, as well as past committee member for multiple NASW-Louisiana committees. Donna is a current member of the LABSWE Rules Committee. Donna has vast experience authoring and presenting continuing education programs across the state.
